Before you download the cover sheet, we suggest that you read the section called What We Fund and/or try out our Quick Check.

 

Get Adobe Reader

Adobe Reader Required

To download a cover sheet and fill it out, you will need Adobe Reader.  If this is not already on your computer, it can be downloaded from here. Simply fill out the form and click the "Download" button at the end of it. Once the download is complete install the Reader by following the instructions provided by Adobe.

 


Cover Sheets - also called Application Forms


  • Cover sheet (229 KB)  If you are having trouble downloading or printing this, you may need to download and install a more recent version of Adobe Reader than the one currently on your computer - see preceding paragraph.  Still no good?  Try the following version in Microsoft Word. 
  • Cover sheet - Word version (69KB) Please ensure that when you print the first page, it still fits on one page (it can get longer if you put too much information in).
The cover sheet also includes a list of the items we ask you to send in with it.

The form can be filled out on your computer by tabbing through the boxes provided.  Please print it out, sign and date it, and return it to us by post or courier with the other items requested. Please do not send it back by email - it is easier for us to get everything together.
